Option B – Make checks out to “Cycle of Influence”, with fee amount correlating to due date,
and mail it to the address below. Mail must be postdated no later than paid fees scheduled end
date. Checks will not be accepted after October 1.

EVENT INFORMATION:
Your Registration Includes: Full Conference access including keynote speeches, breakout sessions,
special events, sponsors (swag) bag. Meals include Lunch, Dinner for 2 days. Breakfast is part of a
Special COI Conference Fee charged by the primary and secondary accommodations providers
(PlumpJack, Squaw Valley Lodge). Conference attendees who make accommodations other than at the
Primary or Secondary resort will be required to purchase breakfast at PlumpJack. Other hotel/resort
fees, taxes and incidentals are not included.

TERMS & CONDITIONS:
• The Cycle of Influence is produced by Smith Smith & Smith, Inc., principals, employees or agents and any other persons or entities acting on their behalf, and the successors and assigns for any and all of the aforementioned persons and entities hereinafter collectively known as COI.
• Payment of registration fees constitutes your agreement to be bound to the Term & Conditions set forth here and to any changes posted to the COI.com web site.
• All cancellations must be received via fax or post, in writing and signed.
• Cancellations before 8/7/09 receive a 75% refund of registration paid.
• Cancellations before 9/18/09 receive a 50% refund of registration paid.
• No cancellation refunds will be given for registration paid after 9/18/09.
• No registration fee refunds will be given for cancellation of the event due to acts of God, fires, labor strikes or any other emergencies that prevent the event to be held under safe conditions. COI reserves the right to cancel this event upon their own discretion.
• Substitution of registered guests are accepted until 10/9/09, however rate differences will apply.
• COI is not liable fo any cancellations for any reason or additional fees by air, ground transportation, rental cars or lodging agents.
• Your submission(s) to “CYCLING WORDS of WISDOM, ADVICE and EXCUSES” hereby grants unlimited license to COI. COI reserves the right to publish writings you have offered for submission. |
• By submitting, posting, publishing, uploading, or distributing any messages, data, information, text, graphics, links, images, content, or other materials (collectively known as "Postings”) to any COI controlled media or dialogue, including but not limited to www.cycleofinfluence.com, Facebook, Twitter group, or the Conference itself, you hereby grant to COI, (or warrant that the owner of such rights has expressly granted to us) a perpetual, irrevocable, royalty-free, non-exclusive, and transferable license to use, reproduce, publish, store and publicly display the Postings in any form. Additionally, you hereby release use of any photographic or video images of yourself under the same conditions as Postings.
• Limitation of Liability and/or Waiver: Guests agree to waive, release and discharge COI of all claims, causes of action, including but not limited to claims or suits for personal property, personal injury, wrongful death, injury to others and/or third parties, loss, theft, and/or damage which arises from registering, attending and/ot participating in COI and/or the use of www.cycleofinfluence.com, including any and all claims related to the sole or partial negligence of COI. Guest voluntarily agrees, understands and recognizes that the Guest will have no right to make a claim or file a lawsuit against COI.
• Severability: Each provision of this Agreement, including any exclusions or limitations of liability, shall be construed separately, applying and surviving even if any provision in this Agreement is held to be inapplicable or unenforceable under any circumstances.
• COI shall have the right to revise and modify the Terms & Conditions at any time by posting the revised or modified T&C on the COI web site
• Terms & Conditions may change.
